BRAF/MEK venøs tromboemboli

NBV for BRAF/MEK udløst venøs tromboemboli

Dyb venetrombose (DVT) kan opstå hos 4-5% og lungeemboli (LE) hos 2-3% af patienterne i MEKi monoterapi eller kombinationsbehandling med BRAFi/MEKi, dog ses det kun hos 0,9% af patienterne i monoterapi BRAFi.1-6 Denne risiko ligger meget tæt på incidensen ved at have en cancersygdom.7

Grad 1

Medicinsk behandling ikke indiceret – f.eks. overfladisk tromboflebitis

Grad 2

Medicinsk behandling indiceret – f.eks. ukompliceret dyb venøs trombose

Grad 3

Akut medicinsk behandling indiceret – f.eks lungeemboli eller intrakardiel trombose

Grad 4

Livstruende konsekvenser – f.eks. hæmodynamisk instabil patient

Udredes for venøs tromboemboli i henhold til gældende lokale retningslinje.

Såfremt udredningen påviser venøs tromboemboli, behandles denne i henhold til lokal retningslinje. Nedenfor angives håndteringen af BRAFi/MEKi ved mistænkt BRAFi/MEKi induceret venøs tromboemboli.

Grad 1

BRAFi/MEKi kan fortsætte

Grad 2-3

OBS gælder kun binimetinib/encorafenib

  • Pausér binimetinib, og reducér encorafenib.
  • Ved bedring til grad 0 eller 1 genoptages binimetinib i reduceret dosis og encorafenib fortsætter i reduceret dosis
  • Ved manglende bedring skal encorafenib og binimetinib seponeres permanent

Grad 4

OBS gælder både encorafenib/binimetinib og dabrafenib/trametinib

  • Seponér både MEKi og BRAFi permanent
